 Ages and Stages Questionnaire

Because your child's first five years of life are so important, we want to help you provide the best start for your child. As part of the CFCE Grant Services we are pleased to now offer the Ages and Stages Questionnaire (ASQ.)

How do I complete the ASQ?

You will be asked questions about your child, and asked to respond to each question with "yes," "sometimes," or "not yet." The questionnaire includes questions about how your child is doing in important areas such as speech and language, physical ability, performance of daily activities (eating, sleeping,) independence, expressing feelings, and how they interact with others. The questionnaire can either be completed at home in 15-30 minutes and you can try fun and simple activities with your child to answer the questions or simply ask one of our program facilitators to fill one out with you and your child at one of our programs.

What is the Ages and Stages Questionnaire?

As Parents and caregivers we all wonder how our children are growing and developing, The ASQ is a simple screening tool to help you keep track of your child's growth and development. When completed more than once over a period of time, the results of the questionnaire become a powerful tool to measure improvement in areas of concern, as well as, skill mastery. We are also now offering the ASQ SE Questionnaire which allows you to monitor your child's social and emotional development.

What happens when I have completed the ASQ?

After you complete the questionnaire, a trained professional will review it and share the results with you. If your child has trouble with some skills we will help you with the next steps. Finding delays or problems as early as possible supports young children's healthy development! Then revisiting the questionnaire in a few months time can be used as a powerful tool to measure improvement and mastery of skills previously assessed!

The ASQ is available for families with children between 4 months and 5 years of age.
